The Prevention and Enablement Model is a whole system approach to improving the lives of people living with disabilities and/or long-term health conditions. Its overarching theme is to encourage and support people to be more active, independent and well. This test and learn is being delivered via a system of unique partnerships across the county’s Health and Adult Social Care sector.
